I suppose you are NOT using J1.5 version ...

Yeah, there's a known problem with custom error page and Joomla 1.6/1.7/2.5, due to changes in the way now Joomla manage errors.

I will fix in the next version, but menawhile you can solve the problem easily, as described here:
